911 Disability Indicator Form

| | Comments (0) | TrackBacks (0)

If someone in your household has a disability (mobility, hearing, speech or sight), you may want to consider filing a 911 disability indicator form. The information provided on the disability indicator form enables a special code to appear on the 911 call takers screen which alerts the call taker that a person residing at that address may require special assistance during an emergency.

A new Disability Indicator Form should be submitted for the following:

1.      a person moves or no longer resides at that address

2.      the apartment number changes

3.      the telephone number changes

4.      to add or delete a disability

You can download a copy of the form or find out more information on the state’s website at: http://www.mass.gov/?pageID=eopsterminal&L=5&L0=Home&L1=Crime+Prevention+%26+Personal+Safety&L2=Report+an+Emergency&L3=Enhanced++911&L4=About+E9-1-1&sid=Eeops&b=terminalcontent&f=setb_disability_indicator&csid=Eeops
